Call the office
Phone is the fastest way to reach us. Call (603) 669-7999 during office hours (Mon–Fri, 10:00 AM – 5:30 PM). A real person will help you set up an evaluation.
Share basic info
We'll ask for your name, date of birth, insurance, and a brief note on what's bringing you in. If you've seen another psychiatrist, please bring or have records sent ahead.
Initial evaluation (~60 min)
Your first visit is a longer appointment so we can take a careful history. We'll discuss your goals, talk through any treatment options, and decide together on next steps.
What to bring
Come prepared so we can spend the visit on you.
- Photo ID and current insurance card
- List of current medications and doses (including supplements)
- Names and contact info for your primary care provider and therapist (if any)
- Records or notes from prior psychiatric care, if available
- Completed intake questionnaire — fill it out online and download as PDF (see below)
Complete your intake form before your visit
Fill in your medical history, insurance, and background online — then download a PDF to bring with you. Takes about 10 minutes and means more of the appointment is spent on you.
Telehealth for follow-up visits
Telehealth appointments are available for established patients. All initial evaluations must take place in person at the Bedford office.
Please don't share PHI by email or text.
